The Oregon Office of Rural Health at OHSU is seeking nominations for the 2017 Oregon Rural Health Hero of the Year Award. The award will honor an outstanding person, program or organization that has improved the quality and availability of health care in rural communities.

"Rural Oregon is full of heroes,” says Scott Ekblad, director of the Oregon Office of Rural Health. "This is your chance to make sure they get the recognition they deserve. Throughout our communities, there are heroes – individuals and organizations – that are making sure that health care remains focused on people. These are the heroes that make our communities the great places to live and are the people you can honor by nominating them for the Health Hero of the Year.”

The 2017 Oregon Rural Health Hero will be announced at the 34th Annual Oregon Rural Health Conference, to be held October 18-20, 2017, at the Sunriver Resort, Sunriver, Oregon.

Nomination forms will be accepted through Friday, June 23, 2017.
